Reducing the intensity of positive pressure ventilation, to mitigate lung injury, remains a major challenge in intensive care—especially when pulmonary physiology is so deranged that conventional protective ventilation cannot achieve safe tidal airway pressures. In this setting, for selected patients, ECCO₂R can support a lung-protective strategy or, in some cases, avoid the need for mechanical ventilation altogether.
